Information Request Form

Many common reference questions may be answered using the Online Medical Reference System, Database Information, Tipsheets and other resources available from the Dykes Library home page. For information requests which require the assistance of a reference librarian, please use the form below to send us your request. Every effort will be made to answer questions on the day that they are received. Questions received after 5:00 p.m. on weekdays may be answered the next day. Questions received on weekends or holidays will be answered on the next business day.

If your question requires in-depth research to find information, there is a $25 per hour search fee. A librarian will contact you before proceeding with any in-depth research.

NOTE: The medical and health information resources of the A.R. Dykes Library of Health Sciences are intended for use by the general public and health care professionals. Staff are prepared to assist in locating information. The Dykes Library staff statements and representations regarding the information found does not constitute medical advice or a recommendation regarding health care or diagnosis.
